As a Senior Backend Engineer, you'll architect scalable solutions that handle sensitive healthcare data with 99.9% uptime requirements while serving millions of patient interactions. The technical challenges you'll solve

You'll design and build systems that tackle unique engineering problems:

  • Scale healthcare interactions from thousands to millions while maintaining sub-second response times on complex medical data queries
  • Architect fault-tolerant systems for mission-critical healthcare workflows where downtime directly impacts patient care
  • Build secure data pipelines that process highly sensitive healthcare records while maintaining HITRUST certification
  • Design automated scaling solutions that handle unpredictable traffic patterns in healthcare environments
  • Optimize database performance across both relational and NoSQL systems, managing massive patient datasets
  • Implement monitoring and alerting strategies that ensure 99.9% uptime for business-critical systems

Working with healthcare data, you'll solve complex challenges related to patient population selection algorithms, care prioritization systems, and cost-effective data enrichment solutions that directly impact patient outcomes.

Your role and impact

You will:

  • Own end-to-end system design for services powering Wellth's mobile app and business-critical healthcare systems
  • Lead operational excellence initiatives, including incident response, post-mortems, and reliability improvements
  • Architect scalable solutions that grow with our expanding patient population and data requirements
  • Drive developer experience improvements through shared tooling, documentation, and SDLC process optimization
  • Mentor and guide other engineers, amplifying team capabilities and technical standards
  • Ensure data security and compliance following secure SDLC guidelines for HITRUST certification
Core technical requirements

We're looking for an engineer with:

  • 5+ years building production services with Node.js and TypeScript at scale
  • 5+ years of operational experience troubleshooting, monitoring, and optimizing production systems for reliability and performance
  • 3+ years with cloud infrastructure (AWS, GCP, or Azure) using Infrastructure as Code (Terraform or CloudFormation)
  • 5+ years working with databases, including both relational and NoSQL systems, with experience in query optimization and scaling strategies
  • Strong collaborative experience contributing to shared codebases, implementing developer tooling, and knowledge sharing through documentation and technical presentations
  • Proven track record solving complex technical problems with reliable, scalable solutions
Operational excellence and scalability experience

Your background should include:

  • Production monitoring expertise with APM tools (DataDog, New Relic, or similar)
  • CI/CD pipeline design and deployment automation (GitHub Actions, CircleCI, or Jenkins)
  • Pub/sub architecture experience with message queues (RabbitMQ, Kafka, Amazon SQS)
  • Search and analytics platforms (ElasticSearch, Solr, or alternatives)
  • Data warehouse experience (Redshift, BigQuery, Snowflake)
  • Security-focused development, especially with sensitive data in healthcare, financial, or government contexts
  • Testing expertise in unit and integration testing for critical system validation
Preferred qualifications

We'd love to see:

  • Healthcare industry experience working with medical data and compliance requirements
  • Data orchestration tools experience (Airflow, Fivetran, or similar)
  • API development expertise in both RESTful and GraphQL architectures
  • AI-assisted development experience using tools like Claude Code, Cursor, Gemini CLI, or similar AI code assistants for enhanced productivity
  • Agile development experience in Scrum or similar methodologies
  • Container orchestration knowledge (Kubernetes, ECS)
